The Service is intended for use by patients, pet owners, and licensed healthcare providers, including veterinarians. You must be at least 18 years of age to use the Service. You also agree to provide accurate, current, and complete information when interacting with BlendRx, including when submitting prescriptions or personal health information through our online forms.
2.1 Submit Rx Form (For Providers)
By submitting a prescription through our online "Submit Rx" form, you represent and warrant that: - You are a licensed healthcare provider authorized to prescribe medications in California. - All submitted prescriptions are issued for a valid medical or veterinary purpose.
- If the patient is an animal, a valid Veterinarian-Client-Patient Relationship (VCPR) exists. - The patient or pet owner has freely chosen BlendRx as their preferred pharmacy. - You will furnish supporting documentation upon request, if needed for verification. Submitted prescriptions may be stored electronically or printed for processing and verification.
2.2 New Patient Form (For Patients or Pet Owners)
By submitting the New Patient Form, you:
- Authorize BlendRx to contact your current or previous pharmacy to request prescription transfers. - Consent to our collection and use of your personal and medical information for prescription fulfillment and identity verification.
- Acknowledge that all data submitted will be handled in accordance with applicable HIPAA and California privacy laws.
3. Data Access and Security
Access to form submissions and protected health information (PHI) is limited to BlendRx pharmacists and authorized personnel (pharmacy technicians, interns, and clerks) only when required within their professional duties. BlendRx maintains administrative, technical, and physical safeguards to ensure data security.
4. HIPAA Compliance
BlendRx complies with the Health Insurance Portability and Accountability Act (HIPAA), California medical privacy laws, and pharmacy board regulations. We protect your health information with secure data handling practices and access restrictions.
A separate Privacy Policy and HIPAA Notice of Privacy Practices will be published and incorporated into our Service.
5. Account Security
If you create an account on our website, you are responsible for maintaining its security. You agree to notify us promptly of any unauthorized use or suspected breach. BlendRx may suspend or terminate access to any account that is used in violation of these Terms or applicable laws.

13. Ryan Haight Online Pharmacy Consumer Protection Act Notice
This online pharmacy will only dispense a controlled substance to a person who has a valid prescription issued for a legitimate medical purpose by a licensed practitioner. This requires at least one prior in-person medical evaluation or a telemedicine evaluation in accordance with applicable law.
